Q7|How to Use? (Clinic Microneedling SOP — Detailed Version)

In 2013, the Nobel Prize in Physiology or Medicine established the cellular “vesicle trafficking” mechanism—essentially the instruction manual for how cells package and deliver signals. That insight helps us understand and modulate exosome formation and release, opening possibilities in skin and aesthetic care. Exosomes have since been widely explored as one of the most promising ways to break long-standing bottlenecks in regeneration and repair.

TL;DR (one-line workflow)

Cleanse & prep → microneedling 0.25–0.5 mm in cross-hatch passes → reconstitute & apply one full vial → 10–15 min calming mask → same-day moisturizer + broad-spectrum/physical sunscreen.

Needle depth, pass count, and coverage are set by trained professionals based on client skin and tolerance.

01|Pre-screen & room prep (T–15–30 min)

Exclude/defer if: active dermatitis/acne flare, open wounds, recent high-energy laser/medium-deep peel, recurrent cold sores (unmanaged), keloid tendency, isotretinoin PO ≤6 months, pregnancy/lactation (use caution/avoid), bleeding disorders/anticoagulants, or any condition judged unsuitable by the practitioner.

Baseline photos: front, L/R 45°, same light/distance/camera settings.

Skin prep: makeup removal → gentle cleanse → wipe with 70–75% alcohol or povidone-iodine avoiding mucosa → dry.

Devices & disposables: sterile single-use needle cartridges/roller heads, saline, sterile swabs/gauze, calming sheet mask, gloves/masks/caps, sharps container.

Product check: EXO-Lyospheres™ freeze-dried vial + dedicated diluent (Pro = standard full-face dose; Max/Ultra = for multi-session plans). Verify LOT / Vial-ID and dates.

02|Suggested microneedling parameters (by facial zone)

Use even erythema/occasional pinpoint bleeding as an endpoint. Do not “chase redness.”

Zone

Depth (mm)

Direction & passes

Forehead

0.25–0.50

Vertical / horizontal / diagonal, 6–8 passes each, light pressure

Cheeks/zygomatic

0.30–0.60 (texture/post-blemish: up to 0.8 as protocol allows)

Vertical / horizontal / diagonal, 6–8 passes each

Nose bridge/alae

0.20–0.30

Light pressure, fewer passes; avoid telangiectasia

Upper lip/marionette area

0.20–0.30

Sweep along wrinkle direction, light

Jawline/nasolabial area

0.30–0.50

Moderate pressure, even coverage

Periorbital (lateral only)

≤0.25

Keep away from lid margin, light tapping

Technique tips: cross-hatch (vertical → horizontal → diagonal), straight in/straight out to avoid drag; wipe micro-ooze between zones; avoid overworking.

03|Reconstitution & application (the golden

0–60 min)

Reconstitute (in-procedure or immediately post-procedure): add diluent down the vial wall, gently invert until clear (avoid vigorous shaking/foaming).

Dose (full face): 1 vial = 1 standard session. For extended areas (face+neck) you may require 1–1.5 vials per session per clinic protocol.

How to apply

  • During passes: a light film can reduce friction (“treat-as-you-go”).

  • Immediately after: dense dotting + light pressing; prioritize ROI (cheeks, nasolabial edges, jawline).

Calming occlusion: apply a cooling/calming sheet 10–15 min to reduce tightness/heat.

Finish: do not rinse. Seal with a barrier-support moisturizer; apply broad-spectrum/physical sunscreen before discharge. Avoid heavy makeup the same day.

04|Client take-home: 72-hour aftercare timeline

  • D0 (evening): no hot water on face; optional saline mist + a light layer of the freshly reconstituted exosome solution (if split use was planned).

  • D1–D2: amino-acid cleanser (<30 s); exosome layer AM/PM; ceramide/squalane repair cream; strict shade/physical SPF.

  • D3–D4: add HA/polysaccharide hydrators; mild itch/tightness is common—persistent redness/ooze → clinic review.

  • D5–D7: optionally re-introduce gentle peptide serum at night; postpone strong acids/high-% vitamin C/strong retinoids for ≥7–10 days or per tolerance.

Cadence: every 7–10 days; 3 sessions ≈ one condensed cycle (Ultra maps to D0 / D7–10 / D20±).

05|Pairing with other procedures (field-tested order)

  • Fractional/ablative lasers & deep peels: device first, exosomes after; for higher energy, wait 24–72 h before topical use.

  • Injectables (HA/botulinum): injectables first; avoid massaging over injection points same day.

  • Chemical peels: after medium–strong peels, wait ≥72 h before exosome application.

  • Home rollers (≤0.25 mm): only for trained clients; cleanse/disinfect → small exosome layer 1–2×/week; always patch-test first.

06|Hygiene & risk control (non-negotiables)

  • Single-use needle cartridges and caps; follow OEM sterilization replacement rules for pens. Never reuse.

  • One vial = one use. If splitting is unavoidable: seal, refrigerate 2–8 °C, finish within 48 h.

  • Out-of-bounds response: persistent burning, spreading erythema, exudate → stop actives, keep only saline + bland repair, and escalate per clinic protocol/medical referral.

08|Tray checklist (ready-to-go)

  • EXO-Lyospheres™ freeze-dried vials + dedicated diluent (enough for that day’s schedule); calming sheets; barrier cream.

  • Microneedling pen + single-use cartridges; backup manual roller (sterile, single-use).

  • Saline mist, sterile swabs/gauze, medical sheet mask.

  • Gloves/masks/caps, sharps container, table drapes, alcohol/povidone-iodine.

  • Record sheet (LOT/Vial-ID, depths/passes, immediate reaction score).

09|Mapping to our product sets (purchase & repurchase)

  • Pro (1+1): single session / first-time experience.

  • Max (2+2): two-week continuity (D0 + D7–10).

  • Ultra (3+3): 3–4-week condensed cycle (D0 + D7–10 + D20±).

  • Fresh-Batch 48h: we allocate the latest released batch, print per-vial labels, and ship with temperature awareness within 48 h of order.

Follow this SOP to lock the post-procedure window and land three things every time: fresh activity, a clean hygiene loop, and per-vial traceability.
